Retainers after braces: types, wear and care

Why teeth move back

The fibres and bone holding each tooth in place take time to reorganise around a tooth's new position. Until they do, the tooth is under constant gentle pull back toward where it started.

This tendency, called relapse, is strongest in the first months after braces come off, but it never disappears entirely. Teeth continue shifting slowly throughout life, with or without previous orthodontic treatment.

This is why retainer wear is long-term rather than a short phase at the end of treatment.

The two types we fit

Hawley retainers are the traditional design. They are robust, can be adjusted slightly, and last well. The visible wire is the main objection people raise.

Essix retainers are the clear ones. They are near-invisible and comfortable, but they are thinner and wear out faster, and they cannot be adjusted - if the fit changes, a new one is made.

Why we do not recommend fixed retainers

A fixed retainer is a thin wire bonded permanently behind the front teeth. It is offered by some clinics because it removes the need to remember anything.

Klinik Pergigian Genting does not recommend them. Cleaning underneath the wire is difficult, plaque builds up where it cannot be reached, and if the wire debonds without you noticing, teeth move while you assume they are being held. A removable retainer worn as instructed does the same job without those risks.

How long do you wear one?

Full-time wear is usual at first, then reducing to nights only. Night-time wear is generally recommended indefinitely.

The usual pattern is full-time wear for the first six months, apart from eating and cleaning, then nights only from then on.

Patients often ask when they can stop. The honest answer is that stopping entirely means accepting that teeth will move over time. Wearing a retainer a few nights a week is a small price for keeping an eighteen-month treatment result.

Cleaning your retainer

• Rinse it every time you take it out.
• Brush it gently with a soft brush and cool water. Hot water warps plastic retainers permanently.
• Do not use toothpaste on clear retainers - it is abrasive and creates cloudy scratches.
• Soak occasionally in a retainer or denture cleaning solution.
• Keep it in its case when it is not in your mouth.

Never wrap a retainer in a tissue. It is the single most common way they get thrown away.

Lost, broken or no longer fitting

Contact the clinic promptly. Teeth begin moving within days, and a retainer left out for weeks may no longer fit when you find it.

If your retainer suddenly feels tight, your teeth have already shifted. Do not force it - come in and have it checked.

Frequently asked questions

How long does a retainer last? Clear retainers wear out sooner than Hawley retainers and are usually replaced periodically. Bring yours to check-ups so wear can be spotted early.

Can I eat with my retainer in? No, for removable retainers. Take them out to eat and drink anything other than water, then rinse and replace them.

My retainer feels tight after a few nights off. Is that normal? It means your teeth have moved slightly. Wear it as instructed and it will usually settle. If it does not fit or hurts, come in - forcing it can damage both the retainer and your teeth.

Do I need a retainer if I only had braces for a short time? Yes. Relapse is not related to how long treatment took.

Are clear retainers the same as clear aligners? No. They look similar but do different jobs. A retainer holds teeth in position. Clear aligners are a series of trays designed to move them.
