A dental implant replaces a missing tooth with a titanium post placed into the jawbone, topped with a crown. Unlike a bridge, it does not rely on the teeth either side. At Klinik Pergigian Genting, assessment and planning is carried out by Dr Reshnu Suri, who holds a Fellowship in Oral Implantology (FDS-OI).

How an implant works
An implant has three parts:
The fixture - a titanium post placed into the jawbone, taking the place of the tooth root.
The abutment - a connector fitted onto the post once healing is complete.
The crown - the visible tooth, fitted onto the abutment.
Bone gradually bonds to the surface of the titanium post over several months. This is what gives an implant its stability, and it is why implant treatment cannot be rushed.

An implant is not automatically the right answer. A bridge may suit you better if the neighbouring teeth already need crowns. A denture may be the sensible choice if several teeth are missing or if surgery is not advisable.

Who implants suit
Implant treatment generally needs:
- enough jawbone at the site to hold the post, which the X-ray will show
- healthy gums, with any gum disease treated and stable first
- good oral hygiene, since implants can be lost to gum infection much as teeth are
- general health that allows healing
Smoking significantly increases the risk of implant failure. It does not automatically rule treatment out, but it is something to discuss honestly at the assessment.
If there is not enough bone, a bone graft may be needed. Klinik Pergigian Genting refers bone grafting cases rather than carrying them out here.
The process, step by step
1. Assessment. Examination and X-ray to check bone volume, the position of nearby structures, and the health of the surrounding gums and teeth.
2. Planning. Dr Reshnu plans the implant position and explains the timeline and total cost before anything begins.
3. Placing the implant. The post is placed into the jawbone under local anaesthetic. Stitches are usually needed.
4. Healing. Bone bonds to the implant over several months.
5. Fitting the crown. Once healing is confirmed, the abutment and crown are fitted.
6. Review. Implants need ongoing check-ups and cleaning like natural teeth.
Looking after an implant
An implant crown cannot decay. The gum and bone around it can still become infected, and that is the main reason implants fail long-term.
- Brush and clean between teeth daily, including around the implant
- Keep up regular scaling appointments
- Tell your dentist if the gum around the implant bleeds or feels tender
- Wear a night guard if you grind your teeth

Consultation starts from RM25, and a PA X-ray is RM60.

Frequently asked questions
Does getting an implant hurt? Placement is done under local anaesthetic and you should not feel pain during the procedure. Discomfort afterwards is usually compared to an extraction and is managed with ordinary painkillers.
How long does the whole process take? Implant treatment runs over several months, because bone needs time to bond to the post. This is not a delay that can be shortened safely.
How long does an implant last? Implants are intended as a long-term replacement. Longevity depends heavily on gum health, hygiene, smoking and whether you grind your teeth. Your dentist will discuss what is realistic in your case.
Can I have an implant straight after an extraction? Sometimes, depending on the site and whether there is infection. More often the socket is allowed to heal first. The X-ray and examination determine which applies.
Are implants better than a bridge? Not always. An implant leaves the neighbouring teeth untouched, which matters if they are healthy. A bridge may be preferable if those teeth already need crowning. It depends on your case.
Can I get an implant if I smoke? Smoking raises the risk of implant failure. It is not an absolute barrier, but it needs discussing honestly before treatment is planned.
